Client Forms & Policies
If you are a new client, please complete the following intake forms before our first session.
Good Faith Estimate of Services
You have the right to receive a “Good Faith Estimate” explaining how much your mental health care will cost. Under the law, patients who don’t have insurance or who are not using insurance for reimbursement have the right to request a Good Faith Estimate of the expected charges for medical services, including psychotherapy services. This can be requested before scheduling an appointment and includes any non-emergency healthcare services, including psychotherapy services.
In the event you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
